CHRONIC ILLNESSES
We understand that sometimes life throws our students curve balls. Whether it be a short-term chronic illness or long-term one, we have a way to get that information on file so we can help your student with exceptions to the attendance policy during that time. Below you will find the Chronic Illness form that will need to be filled out by you and your student's doctor to have it approved by SHS for exception to the policy. This form is good for one semester at a time. So, you will need to have this completed twice a year for it to be valid.
**Please note that if your student needs to be exempted from physical education or sports based on this chronic illness, you will have to get that information to that teacher or coach as well. Having it on file in the office doesn't mean they have been alerted. So, please give them a copy as well.
